Patient was in 9 medications chronic conditions and 3 OTC...

I was confused with a patient in his profile there were 9 medications for hypertension, diabetes, cholesterol and angina. He was 3 analgesics acetaminophen, ibuprofen, multivitamins, omega-3,6,9. I got confused, Not sure where to start, do you have any suggestion...

if patient is confused using several medications, then try addressing the compliance. If pharmacist is confused than initiate the discussion on drug therapy, eliciting the patient's view and experience of treatment, do med check review to ensure continuity of care or collaborate with prescriber to improve treatment protocol.
