Chapter 58 Spectrum of Antibiotics used in Infectious Diseases Sample Questions

1. MP is a 73-year-old woman hospitalized due to shortness of breath, fatigue and weakness. She has Type 2 Diabetes Mellitus (T2DM) with a recent renal clearance of 55 ml/min. MP just recently had ischemic stroke. Upon assessement at the hospital, doctor confirmed penumonia. She is allergic to penicillin. What is the drug of choice?
  1. A. Amoxiclav + Azithromycin
  2. B. Levofloxacin 500 mg for 10 d
  3. C. Moxifloxacin 400 mg q24h IV for 7 to 14 days
  4. D. Levofloxacin 750 mg for 5 d
Answer
Ans: C
Tips: RxTx/CTC. Pneumonia. Only few fluoroquinolones have indication to respiratory infections. Levofloxacin is ≥75% renally eliminated. On the other hand, Moxifloxacin is <50% renally eliminated thus, it is safer to use in patients with renal impairment. Although Ciprofloxacin can be used, it is not a first line choice because of high resistance with S. pneumoniae.
